Petition to Change Program: This form is used to request Advanced Standing, Transfer Credit, Master’s in Passing, Terminal Master’s, Extension of Time, and Leave of Absence.
Comprehensive Exam Committee Approval: To be completed and submitted to the Department in preparation for your Comprehensive Exams.
Dissertation Proposal: To be submitted to the Graduate School.
Dissertation Reviewer's Report: This form must be received by the Graduate School at least one week prior to the defense date.
Electronic Dissertation Release: To be submitted to the Graduate School by the dissertation submission deadline.

Academic Program Changes: To add major/minor.
Evaluation of Transfer Credit: To be used after courses are taken abroad or at another institution.
AB/MA Course Plan: For approval to apply for the Accelerated AB/MA Arabic program.
